This gives your office staff plenty of information to contact the potential client, schedule a visit, and ask for relevant follow-up information. The process is quick and easy.
What Not To Ask
You know what you don’t need? Their social security number, past medical information, or home address. While this is relevant information, and may be included in a new patient form, it is not required for the initial contact. People may feel uncomfortable putting that much information into cyberspace, or if they see a long web form, they may simply move on rather than spend the time filling it out.
